In 2019, the client conducted an analysis of primary care enrollment and productivity standards aimed at providing input for updating current policies and improving access. PACT is a team-based Patient-Centered Medical Home model that was established in 2010. The model requires support for access and has not adequately studied various staffing and enrollment models. The goal of this project is to use a population health approach to evaluate the outcomes and cost of three PACT staffing and enrollment models. ​

Our team is responsible for the implementation and performance analysis.​

• Baseline evaluation of current PACT enrollment, and performance.​

• Team member responsibilities, workflow and pilot development/implementation.​

• Team member and leadership training.​

• Outcome performance analysis, including cost, population health metrics, access to care, and healthcare consumption measures.
